General Terms & Conditions for Members.

These terms and conditions are applicable to all members of Belper Hockey Club (BHC), comprising Belper Ladies Hockey Club and Belper Men’s Hockey Club, including all Juniors and Mini Hockey.

Declaration for players over 18 

  • I consider myself to be physically fit and capable of participating fully. I agree to notify Belper Hockey Club (BHC) of any changes to the medical information provided. In the event that I am injured I give my permission for the BHC members present to obtain emergency medical treatment on my behalf. 
  • I agree to abide by the BHC Codes of Conduct for behaviour and discipline. These are displayed on the BHC website, and summarised below. 
  • I agree to pay my club fees promptly and understand that any delay may result in my deselection until I have paid. (Members are encouraged to pay by Direct Debit where possible).
  • I agree that photos taken of games/events in which I am shown may appear on the BHC website, or be used for promotional purposes by Belper Hockey Club or Belper Meadows Community Sports Club including social media. 

Declaration for parents or guardians of all players under the age of 18 

  • As the parent or person with legal responsibility for an under 18 player I consent to that child playing hockey with Belper Hockey Club (BHC). I consider that child to be physically fit and capable of participating fully. I agree to notify Belper Hockey of any changes to the medical information provided. In the event that the child is injured I give my permission for the BHC members present to obtain emergency medical treatment on their behalf. I am aware of the nature of the game and whilst I understand every care will be taken, I accept that in the event of an accident I cannot necessarily hold BHC responsible. 
  • I agree my son / daughter will abide by the BHC Codes of Conduct for behaviour and discipline. These are displayed on the BHC website. 
  • I agree to that child being transported to and from matches and training in private cars. 
  • I am aware that it is a requirement when taking part in a practice/training or game for my son/daughter to wear a gum-shield and shin-pads at all times. 
  • In some environments, particularly adult competition it is impossible to control photography by external parties, however, I am aware that there may be times that photographs and/or footage maybe taken during matches and training sessions by approved persons or officers of BHC. Such images shall only be used for publicity/training purposes in accordance with England Hockey guidelines and I give consent for the player to feature in such photos/ images on the BHC website, or be used for promotional purposes by BHC or Belper Meadows Community Sports Club including social media. 
  • Belper Hockey Club strives to provide an environment where everyone feels safe and comfortable. We have had good success in integrating the juniors into the adult squads, and to ensure a smooth transition, parents need to be aware that U18 players need to be comfortable and confident about being around adults, both on and off the pitch. 

Belper Hockey Club strives to comply with the high Safeguarding standards set out by England Hockey.

Please note: to revoke the photography consent you must answer ‘No’ to the question ‘Photo Consent? Do you give consent for photographs being taken of you, in line with the EH policy on photography’. This appears in the ‘My Profile’ section the website. Reasonably practicable steps will be taken to inform relevant parties of Belper Hockey Club.

Safeguarding of our Mini Hockey players: Parents/guardians of players must stay on site during coaching sessions/matches.
